Transitions

Forming a lifelong alliance with students and families

From the moment a student enters Crotched Mountain School, our goal is for all students to participate fully in life with dignity, respect and the right to make choices that determine their own future with the fewest restrictions possible.

A team effort in transition planning
Our team of professionals works closely with students, parents, schools and community agencies to create and implement a plan for transition to home, community or a return to public school. Together, we combine the students' interests, skills and goals with education, therapies and activities to prepare for a smooth transition. We help ensure that each student will be welcomed as a valuable member of that community.

A partnership for life
Crotched Mountain School forms a lifelong alliance with our students and families. We offer services, resources and supports throughout all facets of life, including:

  • Assistive technology access and training
  • Community care home-based services
  • Day programs
  • Home evaluations
  • Outpatient services, such as physical, occupational, speech and aquatic therapies
  • Residential services on and off campus
  • Vocational programs
  • Wheelchair clinic
