Make A Referral

Healthcare professionals interested in referring a patient are encouraged to contact our nurse referral coordinator and supply the following patient information:

• Demographic info
• Latest history and physical
• MD/APRN progress notes (from past 7 days)
• Initial therapy evaluations and updated therapy notes
• Nutrition notes
• Most recent lab test results
• Current medications
• Any infectious disease precautions
• Most recent nursing notes (from past 7 days)
• Pertinent consult notes
• Trach type (if applicable)
• Height and weight
• Wound info (if applicable)
• Vent info (if applicable)

Insurance

Crotched Mountain Specialty Hospital holds contracts with most major insurance companies and continues to add more, including:

• Anthem/Blue Cross
• CIGNA
• Coventry
• Harvard Pilgrim
• MVP Healthcare

In addition, we accept patients on a per-client-approval basis from MassHealth, New Hampshire Medicaid, and other state Medicaid plans, Medicare, and many private insurance plans.
