JOB QUALIFICATIONS:

· High School graduate or equivalent, current licensure in the state of New Hampshire as a Certified Nursing Assistant.

. Able to perform/participate in the following activities:

· Independent movement/transfer of patient/client weighing up to 50lbs 30% of the shift;

· Assisted movement/transfer (2 or more person) of client weighting up to 25lbs 10% of the shift;

· Lifting/carrying of objects/items weighing up to 25 lbs. 10% of the shift;

· Standing/walking 80% of each shift;

· Climbing stairs 20% of each shift.

JOB DESCRIPTION:

Basic function is to instruct, supervise, and assist residents with routine care and activities of daily living, and to monitor and provide for the medical needs of the residents in accordance with individual education plans and Center policies/procedures.

Performs tasks under the supervision/direction of Program Manager, Assistant Program Manager and Head/Charge Nurses in accordance with established Center policies/procedures.
