PORTSMOUTH, NH Ann Schwartzwalder, MSW of Kittery, ME has been named program director for Crotched Mountain Community Care.
Ms. Schwartzwalder joined Crotched Mountain in 2000 as a care manager. She received her master's degree in Social Work from the University of Connecticut.
"Having been part of our team for six years, Ann has earned the respect of her colleagues and the trust of our clients and their families," remarked vice president for community services Peggy Lins. "We welcome her leadership as we strive to improve the lives of the people we serve."
Since 1986, the Community Care program has offered an alternative to nursing home placement for New Hampshire adults with chronic illnesses or disabilities who choose to remain in their own homes. The team of care managers coordinate services provided by local agencies, government programs, and/or community organizations, and advocate for those in need of resources or funding to ensure independence, safety, and quality of life. With regional offices in Portsmouth, Rochester, Manchester, Keene, Claremont, and Whitefield, Crotched Mountain serves more than 700 Community Care clients statewide.
Crotched Mountain is a charitable organization employing more than 900 people. Its mission is to serve individuals with disabilities and their families, embracing personal choice and development, and building communities of mutual support. Crotched Mountain provides specialized education, rehabilitation, community, and residential support services for more than 2,000 consumers, including individuals with disabilities and the elderly, living in New England and New York.
