Caring for a relative or a loved one at home can be an overwhelming task. That's why Crotched Mountain began our Community Care program in 1986. We have over 20 years experience providing quality support, advocacy, and community connections to residents of New Hampshire, including caregivers, their family members, and individuals who are seeking to achieve and maintain a safe and healthy lifestyle at home in their community. Our care management services include:
- in-home assessment
- development of a plan of care based on your needs
- referrals to home care agencies to provide the care you need to continue living independently and safely in your home
- ongoing monitoring of care plan
- ongoing advocacy and care coordination with the home care agencies providing your care, your physician and any other community resources needed to meet your needs
- arrangement for transportation to medical appointments as needed
- we serve as a liaison to your family who may live at a distance and keep them updated and informed as to your well being
- assistance with applications and benefit management when necessary including state assistance programs
- ongoing support, advocacy and community connections to you and your family
- a sliding fee scale based on income
Our team of knowledgeable and compassionate care managers can provide peace of mind for you and your family. We can connect you to the community resources and providers needed to enable you to live safely and independently in your own community.
Individuals who are eligible for the New Hampshire Home and Community Based Care for the Elderly and Chronically Ill (HCBC-ECI) Medicaid program may be able to obtain services at no cost. Depending upon your level of income, consumers may be expected to share the cost through a direct payment to the State. For individuals who do not meet the HCBC-ECI criteria, Crotched Mountain accepts private payments through a sliding fee scale based upon income.
