Since 1986, Crotched Mountain Community Care has provided home care services and care management for New Hampshire residents, 19 years and older, with chronic illnesses and disabilities. Our goal is to assist our clients in their desire to create a lifestyle that enhances independent living, while ensuring the highest quality of care is given to each client.

This program offers an alternative to nursing home placement for people who wish to remain in their own homes. Our care managers begin with an assessment of each person's and their family's needs and then develop a cost-effective, individualized independent living plan that meet those needs. The care manager then coordinates services provided by local agencies, government programs or community organizations to supplement care provided by family, friends, and neighbors.
