W. Carl Cooley, MD, Medical Director, is a developmental pediatrician with fifteen years' experience in the study of primary care office systems and strategies for change and improvement with an emphasis on medical home transformation. Dr. Cooley is also Chief Medical Officer for the Crotched Mountain Foundation in Greenfield, New Hampshire, and adjunct professor of pediatrics at the Dartmouth Medical School. Prior to his specialization in developmental pediatrics, Dr. Cooley spent eleven years in community-based, primary care pediatric practice. Dr. Cooley chaired the first national Medical Home Learning Collaborative and serves on the Medical Home Program Advisory Committee of the American Academy of Pediatrics. In 2006, he was the first recipient of the American Academy of Pediatrics Calvin S. J. Sia Community Pediatrics Medical Home Leadership and Advocacy Award. Jeanne W. McAllister, RN, MS, MHA, Director, is Director and Co-Founder of the Center for Medical Home Improvement (CMHI), Crotched Mountain Foundation, based in Concord, New Hampshire. She is an Adjunct Assistant Professor of Psychiatry & Pediatrics at Dartmouth Medical School in Hanover, NH. Ms. McAllister's work focuses on the implementation of the community-based primary care medical home, the quality improvement team processes needed to develop primary care medical homes, the measurement of its quality and value as a fundamental resource for children, youth, adults, and families, and the necessary policy changes to support and sustain its development. She is co-designer of CMHI's Medical Home Index and lead consultant to the American Academy of Pediatrics in their development of a toolkit preparing pediatricians to fully implement the medical home and achieve NCQA recognition in the process. Care delivery management, including care coordination, is critical to a highly performing health care system. In her work supporting the medical home Ms. McAllister has developed practice-based materials, which support primary care teams to implement care coordination - including assessment, care planning and educational tools. She is co-author of a Commonwealth Fund Care Coordination Brief. Lori Keekl-Markowitz, RN, BSN, is the Program Manager at CMHI. Her role at CMHI consists of providing practice support for primary care medical home improvement activities, technical assistance, medical home education and training, the development of medical home tools and materials, and the fiscal management of CMHI's grants and contracts. She brings to the CMHI team over ten years of experience implementing clinical and service quality improvement projects in adult and pediatric Primary Care Practices and Specialty Care Practices. Leah Reed is the Program Coordinator for CMHI. In her role, Leah provides administrative supports for CMHI's medical home improvement activities. Leah has seven years experience empowering caregivers whose children have serious emotional and behavioral disorders to become full partners in their care. In addition, she was the primary caregiver for her father-in-law, who had a multitude of health issues, for six years while he lived with her and her husband. Leah holds an Associates of Science in Human Services degree and brings experience with the delivery of community based training programs. A strong skill set includes event planning and scheduling, resource organization, and outreach to families and professionals linking them with pertinent resources. Lora Council, MD, MPH - Quality Improvement Consultant is a Family Medicine physician with Dartmouth-Hitchcock in Hudson, NH. She completed a residency in Family Medicine at the Concord Hospital Family Health Center and in Preventive Medicine with the Dartmouth-Hitchcock Leadership Preventive Medicine Residency. Dr. Council works with CMHI on improving the structures and functions in primary care practices to create excellent medical home care. She is especially interested in interdisciplinary team communication and teamwork as well as care plan development and use. Dr. Council is interested in promoting patient-centered, goal-directed collaborative care plans as a key tool in accomplishing the aims of the medical home.
