RESIDENTIAL SERVICES

Providing residential and day programs, since 1984, for adults with
developmental disabilities and acquired brain injuries throughout
southeastern New Hampshire and northeastern Massachusetts.

A CHOICE OF SERVICES

We develop our residential and day programs around the unique needs and interests of each person we serve. Every day is an opportunity to grow as a person, learn new skills and experience different aspects of home and community life. With staff support, consumers develop skills in areas such as self-care, communication, socialization, relationship building, problem solving, as well as money and medication management. They also participate in community and recreational activities with friends, family and Crotched Mountain staff members.

RESIDENTIAL PROGRAMS

  • Community Residence: Consumers (2-4 people) live in a private home in the community with Crotched Mountain staff providing 24-hour/7-day support.
  • Individual Service Option (ISO): The consumer lives in the home of a "provider," a local person or family contracted (not employed) by Crotched Mountain to provide needed support. Our role is to over-see the provider and the program.
  • Companion Employee: The consumer lives in his or her own home or apartment with a live-in companion (person or family) employed by Crotched Mountain to provide needed support.
  • Apartment Setting: The consumer lives in an apartment, either alone or with a roommate, and receives support from Crotched Mountain staff for a few hours each day, as needed.

DAY PROGRAMS

  • Day Habilitation: Based upon the consumer's personal interests, Crotched Mountain offers access to many non-vocational community activities such as volunteering, taking a class, or enjoying a recreational day trip. Services are offered Monday through Friday, usually for 6 hours each day.
  • Family Support: We offer assistance in daily living to consumers and their families developed around their particular needs, including personal care, household chores, and transportation. Services are based both in and out of the family home and are offered Monday through Friday, usually for 8 hours each day.

OUR COMMITMENT

To provide support for those seeking services in a manner that makes dignity, respect, and the right to make choices and determine one's future the highest priority. Crotched Mountain will support those who wish to take risks that lead to growth and to the fullest experience possible in the various domains of life: home, work, recreation, relationships, the pursuit of spiritual needs and the right to dream.
