Applications are currently being accepted for review at the April 8th, 2008 meeting but must be received here by the end of the business day on March 3rd, 2008. These deadlines are firm due to the great number of applications we receive. Please keep the following in mind:

• l A separate physician's diagnosis of your disability and recommendation of the request is a requirement. Your application will not be complete without it.
• l Please be sure to include the income from all the adults in the family from all sources. Applicants are considered minors through age 20, so the income of the parents of anyone age 20 or under must be included. Verification may be requested. Requests from applicants age 25 and under are a priority of the HGF Trustees.
• l Please complete all requested information on the application front and the narrative on the back. An application cannot be processed until it is complete (all requested information provided.)
