If you have not already done so, please submit the following by email attachment:
a) A copy of your C.V. or resume which includes your highest education level completed and which states your intended academic major
b) A copy of the biographical pages of your passport (must be legible) Please make sure that your passport is valid for at least six-months after your scheduled return date home at the end of the training program.
c) A recent grade transcript in English.
If any document needs to be translated into English it must be accompanied by the translation form located here:
If you have not already done so, please submit the following by regular or express mail:
d) An original, signed, Crotched Mountain application
e) Three letters of reference (must be either in English or translated into English)
f) An original Police Record Check (must be either in English or translated into English)

It is recommended that you keep a copy of each form for your own reference. The Vacation Calendar shows the School Vacation Periods at Crotched Mountain. We release all participants from placement during these periods and encourage you to use this time for travel within the USA. Please note that there is no expense stipend payment during these periods, though you will still be charged for your accommodation. Please also note that actual vacation dates vary depending on whether assigned to the Monday-Thursday or Friday-Sunday schedule.
Please return a signed copy of the Program Agreement to me along with the $350 payment for your registration to the online course with the College of Direct Supports. Preferred payment is by money order or bank draft (sometimes known as a 'cashiers check') made payable to 'Crotched Mountain', in US dollars only and payable through a US bank. If you wish to pay by credit card please contact the IVS office and we shall put you in contact with our A/P office to make that transaction directly with them. Please also return a completed Travel Form, keeping in mind that it is recommended that you do not purchase travel until after you have obtained your visa from the US embassy.
