GRADE:  C11
SALARY RANGE:  $29,120 - $43,680

BASIC FUNCTIONS:              Perform professional care management services consistent with agency policy, applicable state regulations, and accepted standards of practice.  Provide services to clients and their families in the home and community setting as appropriate.  Determine eligibility of clients for agency programs and services.  Work as a member of the care management team in serving agency clients with continuous networking and community contacts.  Work closely and cooperatively with community agencies providing services to clients and serve as an advocate for clients.

 

SCOPE:          This is an entry level position reporting directly to the Team Coordinator.  Work is performed with self initiative and management in keeping with the standards of excellence set forth by the program.

           

QUALIFICATIONS:            

BS in Social Work, nursing or closely related field with at least two years of experience in human services setting.  Working knowledge of human service resources, knowledge of aging process and issues facing a disabled population.  Good verbal and written communication skills and demonstrated commitment to client oriented human services.

 

SCHEDULE:  During published office hours as established for the program; may require some additional hours as program dictates.

 

PHYSICAL REQUIREMENTS:

·         Physically able to lift various material up to 15 pounds on an occasional basis.

·         Possess dexterity required to frequently operate computer as require by position.

·         Ability to remain seated frequently to continuously much of which in the ability to consistently drive an automobile safely and legally in all local weather conditions 25% of the time on average..

·         Able to make home visits up to 50% of the time, under a wide range of living conditions which may include climbing stairs, climbing hills and standing.

·         Able to perform in-home assessment, including visual observations of clients social, environmental and physical needs.

·         Ability to verbally communicate clearly both directly and by telephone with hearing impaired clients.
