Since 1995, Crotched Mountain has partnered with school districts throughout New Hampshire and neighboring states to support students with neurological, genetic, and behavioral conditions. This includes those with sensory impairments (blind, visually impaired, deaf, hard of hearing, or deaf-blind). Our School Partnership Program (SPP) provides evaluation, education and consultation services on behalf of children and adolescents with special needs in order to assist school teams and families in preparing students for their adult life. SPP conducts the majority of its evaluations in the student's school.
The Evaluation Process
We begin by reviewing the evaluation questions asked by the student's school team, the student's current Individualized Education Plan (IEP), the reports of the most recent triennial evaluation, and medical and other pertinent records. If we are the appropriate resource to answer the questions posed by the school, the next step is to propose an evaluation to the school. Evaluation visits are then scheduled according to the school's request(s) and the needs of the student. One or more of our specialists will travel to the school to conduct the evaluation. Full staff evaluations occur over several visits. This allows for multiple observations and increases the accuracy of our findings.
Following the evaluation, SPP staff reviews its findings and develops recommendations that are based on our extensive experience working with students with disabilities at Crotched Mountain School, and in other schools throughout New Hampshire and neighboring states. An integrated report is then prepared and sent to the school and to the family. A follow-up meeting is held with school staff and the student's family to discuss the report, and to assist the local school team with the implementation of the recommendations.
Consultations and Trainings
SPP specialists are available for follow-up consultation to assist with program development and implementation. We also provide in-service trainings on a variety of special education topics. Funding Options
Many of our evaluations qualify for partial reimbursement through Medicaid for those school districts who have so applied. In addition, schools can elect to pay for our services by using Individuals with Disabilities Education Improvement Act (IDEIA) funding if they include evaluations or staff training in their grant application budget. |
