HORIZONS A NEWSLETTER FOR FAMILIES AND FRIENDS OF CROTCHED MOUNTAIN SPRING/SUMMER 2008, VOLUME 8, NUMBER 2 SPECIAL DONOR RECOGNITION ISSUE A Lifelong Alliance and Legacy for Future Generations by Don Shumway President and CEO CROTCHED MOUNTAIN’S mission to forge lifelong alliances with people with disabilities means our programs and services offer supports to people of all ages, in all phases of life. Our Brain Injury Center (BIC) and residential programs follow people with brain injuries caused by accident, stroke or other circumstance through rehabilitation, discharge and transition to independent or supportive living. In this issue we highlight Peter VanCourt, a former patient at the BIC who has made great strides in recovery from a brain injury he sustained when he was hit by a car in early 2007. As you’ll see in Peter’s story, life continues after formal rehabilitation, as does the need for ongoing supports for many. We have expanded our residential programs to include a number of supported residences statewide and around New England serving approximately 200 people. And we recently opened a day program in a Greenfield town building for 24 adults with disabilities. It’s a place for people to access information and resources, companionship, volunteer activities and community meals. It’s one of many ways we build community in the community. Another community of special importance to us is the natural habitat that surrounds our Greenfield facility. Recently, we’ve undertaken an ambitious project to permanently preserve 1,165 acres through a conservation easement and create an accessible recreation center. An important partner in this endeavor is the New Hampshire Land and Community Heritage Investment Program(LCHIP), which invested $250,000 to match federal funds toward our preservation efforts. LCHIP’s commitment to preserve New Hampshire’s significant natural and cultural places will ensure Crotched Mountain’s meaningful alliance with future generations as well. LCHIP AND CM A PROJECT FOR PEOPLE AND TREES The New Hampshire Land and Community Heritage Investment Program is an independent state authority that makes matching grants to New Hampshire communities and non-profits to conserve and preserve the state’s most important natural, cultural and historic resources. The $250,000 LCHIP award to Crotched Mountain provided all important matching funds for our Forest Legacy Program award, a federal program that so far has allocated just under $1 million. This year our project is ranked 12th in the country in the continuing competitive process, so we hope to receive additional funding for the easement valued at $2.71 million. “It’s a fantastic and deserving project,” said Deborah Turcott, LCHIP executive director. “LCHIP’s mission is to protect land for the good of the people, and this project encompasses both ideas.” CROTCHED MOUNTAIN COMMUNITY CARE Keeping Those Home Fires Burning Brightly JUST BECAUSE we call ourselves Crotched Mountain doesn’t mean everything we do happens on the mountain. Crotched Mountain Community Care(CMCC), which provides care management to older adults and adults with disabilities in their homes, is on the ground serving over 800 people across New Hampshire. Started in 1989 as a pilot project on the coast, the program has expanded to five regional offices whose care managers make it possible for people, who would otherwise live in longterm care facilities, to remain independent in their own homes. Rosemary “Rosie” Collins is one such person. After surgery last year, Rosie spent some time recuperating in acare facility. After discharge she still needed assistance with day-to-day activities and, through an acquaintance, Rosie and herdaughters Anne, Chris and Ellen, got in touch with CMCC director, Ann Schwartzwalder. “We were happy to help,” says Ann. “Our caremanagement services are designed for people like Rosie’s children who are part of what we call the sandwich generation—adults who have younger children at home and who care for aging parents as well.” Ann says CMCC’s care managers act as eyes and ears for family members who can’t be with their parents as much as they want because they work or live far away. In this case, Rosie required daily assistance with household tasks, and due to more recent health concerns, she now receives round-the-clock care coordinated by CMCC care manager Bonnie Konopka. Bonnie and Rosie’s daughter Anne work together to oversee staffing and other services Rosie needs. Rosie’s daughter says the arrangement is working great on every level. “When Mom was in the care facility recuperating, she was unhappy and making little progress. She had clearly expressed to us over the years that her goal was to ALWAYS stay in her own home. We knew she would be happier there. The challenge was to develop the support structure—basically running a nursing home for one,” Anne recalls. “I knew CMCC could help. My sisters and I just couldn’t be there 24/7. Bonnie streamlined the process and made it easy. From staffing, to administrative and insurance issues, to help with care-related family decisions, it was a relief to have an objective, knowledgeable voice in all these matters.” Rosie is somewhat frail, but still enjoys frequent visits from friends and family. “The relationships she has with her caregivers also keep her engaged and interested,”says Anne. Rosie has many people in her life. “Friends and family stop by every afternoon,”adds Bonnie. “And she’s so happy to be able to visit with them in the comfort and safety of her own home.” To learn more about care management services through CMCC, call Ann Schwartzwalder at 603.431.3042. Making a Life After Rehab SOMETIMES IT TAKES A VILLAGE ABOUT A YEAR AGO, Peter VanCourt would be the first to tell you his life was pretty difficult. But he had no idea then how much more challenging his life was about to become. Peter was one of a growing number of homeless employed. He had a job he went to everyday, but when he left work at the end of his shift, he went home to a tent. One cold January evening in 2007, as he was crossing the street, Peter was hit by a car. He sustained a number of severe injuries to his brain, spinal cord, shoulder and legs. “They tell me I was in a coma for ten days,” reports Peter, who still doesn’t remember much about the accident. Peter was hospitalized at Dartmouth Hitchcock Medical Center in Lebanon for four months. At the time of his discharge, he needed a wheelchair to get around and he still had significant cognitive impairments. “When it came time for Peter to leave Dartmouth, he had nowhere to go and still would have benefited from inpatient rehabilitation,” recalls Jenn Bridges, nurse manager on Crotched Mountain’s Brain Injury Center(BIC). Fortunately, the BIC had a bed available and Peter was transported to Crotched Mountain to begin the hardwork of rehabilitation. “When I got here I was pretty confused. It took me awhile to find my way around,” says Peter. But find his way he did. According to BIC assistant director and case manager Belinda Ryan-Magoon, “Credit for Peter’s success is all his. He worked very hard. He ran with every support we had to offer him.” Supports that, among others, included cognitive, physical, speech and occupational therapies. Of all of them, Peter says his favorite was cognitive therapy, which helps people recover functions such as planning, organizing and memory through practical applications. In Peter’s case, cognitive therapy largely involved making furniture—an activity he was familiar with and one he embraced with enthusiasm. “In regaining cognitive skills, we do what makes sense to the client,” explains Tom Badgley, cognitive rehabilitation therapist. “Peter’s a talented guy with lots of practical skills. He’s a welder and a carpenter, so the wood shop was a natural fit.” “It was great. Tom and I would get together and assemble Adirondack chairs to sell in the BIC store. It kept me busy and my mind occupied,” says Peter. According to Tom, the wood shop offered ample opportunity to work on cognitive skills like time management, focus and task completion. “The wood shop became a good anchor for him,” says Tom. Peter also readily reached out to other patients in the center. Belinda recalls he took one gentleman with memory issues under his wing, regularly helping to orient him to activities, people and places throughout the day. Peter speaks affectionately of many of the people he’s lived and worked with at Crotched Mountain. And, since his discharge from the BIC, he now refers to some of them as neighbors and co-workers. Last fall, despite some lingering cognitive and orthopedic issues, Peter was ready for discharge from the BIC. “The problem was he did so well, he wasn’t eligible for many community-based services,” says Belinda. “He’s one of those people who can be independent. He can work, he can keep himself safe, but he still needs some supports.” This seemingly good news resulted in Peter falling through the cracks of the service net for people with brain injuries, but that just motivated the folks in many departments at Crotched Mountain to get more creative. Peter was certainly a candidate to live independently, as long as he could count on regular supports such as help with paying bills, shopping and transportation. An apartment became available on campus and in November Peter moved in. And with the help of the human resource and maintenance departments, he’s now employed by Crotched Mountain as a member of the maintenance staff and currently works in the laundry. Peter gets some of the regular supports he needs from John Martin, a licensed nursing assistant in the center, with whom Peter forged a particularly close bond. “John registered as a volunteer with the Monadnock Volunteer Center so he could continue to help Peter, but as a volunteer rather than a staff person,” explains Belinda. “Crotched Mountain wrapped its arms around Peter,” saysJenn Bridges, pointing out that Peter’s story is not unique, but rather an apt illustration of the way people at Crotched Mountain are committed to the mission of the organization—to offer a life-long alliance to people with disabilities. Peter beams when he talks about his experience at Crotched Mountain, especially when he refers to the help he received from Belinda, who coordinated the many loose ends that had to come together to make his discharge successful. The final loose end was tied up recently when Peter became the first client of the Contoocook Valley Transportation Cooperative, a volunteer group organizing transportation options for residents in the Contoocook Valley. Peter will now be able to get to the grocery store and manage other errands in town when he wants. “I’d be lost without you,” he says to Belinda with a smile. Thinking and Knowing REGAINING COGNITIVE SKILLS WHEN IT COMES TO cognitive rehabilitation therapy, every moment is a teachable moment, according to Tom Badgley,c ognitive rehabilitation therapist with the BIC. “Everything is cognition from the most basic self awareness to decision-making and time management.” So, when it comes to working on cognitive skills with people in the Brain Injury Center at Crotched Mountain, Tom and the other therapists who comprise the Cognitive Rehabilitation Therapy Program don’t have to look too far to come up with the right approach to engage clients. Led by occupational therapist Linda Murphy, the team starts with an assessment to determine a client’s strengths and deficits, and then the work becomes more creative. With some clients, it begins with basic self awareness. A goal might be for them to identify three changes in their lives since their brain injury or to orient themselves to time and place. A memory exercise might entail remembering a therapist’s name or who has visited a client’s room on a given day. The intensive four-day-a-week, two-hour-a-day program recently implemented on the BIC features, among other modalities,a group component. Tom recalls a session where members played an alphabetical memory game listing items from A to Z that Tom should take along on his Bahamas vacation. “It was fun because the things they had me taking along where pretty unusual and memorable. The clients were amazed that with practice they could recall so many items.” Repetition, says Tom, is often the key to regaining skills. Practice does make a difference, as does using compensatory tools such as lists, timers and other strategies to enhance memory, focus and organizational functions. Crotched Mountain is continually challenged to provide this kind of rehabilitative therapy to clients because, even though recovery of cognitive function is necessary for a successful return to independent living, these services are not reimbursed by private insurance companies. “Just as a person with a spinal injury has to relearn how to stand and walk, a person with a brain injury needs to relearn thinking strategies,” says Tom. Crotched Mountain will co-sponsor and host the 15th Annual Conference of the Society for Cognitive Rehabilitation on September 12 and 13. We’re happy to let you know that, at the age of 91, Mr. Kent continues to enjoy good health and recently made the trip to visit us at Crotched Mountain. Mr. Kent’s connection to Crotched Mountain started in 1960, when he was a WBZ-TV weatherman and several of our students joined him on air. For the next twelve years, the “kids at Crotched Mountain” were involved in his weather forecasts, calling their report into the station on a daily basis. And Don Kent’s commitment to Crotched Mountain didn’t end there. He remains an ardent supporter, leaving a legacy that includes members of his TV audience who continue to support Crotched Mountain because of the passion he shared for the many children he met over the years. The Don Kent Legacy Society We’ve recently created The Don Kent Legacy Society to honor those who have included Crotched Mountain Foundation in their estate plans. We Salute Carol and Howard Stoner PARTNERS IN PARENTING, BUSINESS AND PHILANTHROPY CAROL AND HOWARD STONER have known each other all their lives. Born and raised in Montana, their parents—even their grandparents—were friends, so it was no surprise that they became high school sweethearts and married thirty-eight years ago. Their college years found Howard at the University of St.Thomas in St. Paul, Minnesota, while Carol did her undergraduate work at Rutgers University and later received a Ph.D. in biochemistry from Brandeis. In 1981 Howard formed Stoner & Company, which has grown to serve insurance companies and some of the world’s largest corporations when they sustain losses due to natural disasters. “We evaluate, rescue and liquidate damaged merchandise, material and other assets all across the country,” explains Howard. While Howard was building the business and they were starting their family, Carol worked as a scientist at Dana-Farber Cancer Institute and taught at Harvard Medical School. But after their son John was born, Carol joined Stoner & Company, where she now manages the finances. This partnership reinforces their most important job—parenting their four children, Dax, Allison, John and Brendon. “We’ve always shared the day-to-day decisions,” says Carol. When they learned that John had been born with a brain that was not completely developed, Carol and Howard sought out specialists to find the best therapies and care, and to plan for his future. As John grew older, his physical needs became more involved and his inability to speak and communicate clearly often resulted in difficult behaviors. Carol remembers vividly, “It was as if John was the center of a vortex that was pulling us all in.” The decision to enroll John in a special school “was the hardest one we ever made,” recalls Carol. But when they found Crotched Mountain, they realized it was the best possible situation for John and their family. “It has made all the difference,” says Howard, “for John and for us.” “Crotched staff—teachers, therapists and residential aides—worked together on John’s educational needs and behavior issues and today he can go anywhere we go,” smiles Carol. “The first year John was at Crotched,I said ‘I’m going to take a year off from worrying,’ and I’ve been able to take every year off from worrying since then,” she continues. John’s sister and brothers are devoted to him. Brendon even won an award for a paper he wrote in the eighth grade called The Bumpy Ride where he describes a day with John as “the best day of my life.” The Stoners also partner in their philanthropic and volunteer interests. In addition to their church, Chamber of Commerce and many community service involvements in Sudbury and Boston, Howard has served, and Carol is currently serving, on the Crotched Mountain Board of Trustees. They have made challenge gifts to the Board and initiated an annual golf tournament in Massachusetts that realized over $90,000 for Crotched Mountain in its first year. Recently, John graduated from Crotched Mountain and has successfully transitioned to a residence not far from where the Stoners live in Sudbury, Massachusetts. “Crotched Mountain saved our lives as a family,” Carol emphasizes. “We can’t do enough in our lifetime to repay all that Crotched Mountain gave us.” BRIEFLYnoted VOLUNTEERS TAKE A SPRING BREAK AT CM THIRTEEN ALTERNATIVE SPRING BREAK STUDENTS from the University of Illinois at Urbana-Champaign drove 23 hours to volunteer at CM during spring break. Helping out at CM School and the Hayden Day Program, they clocked close to 500 volunteer hours during their stay. They described their experience as “inspirational” and “amazing,” and were thankful to all those who contributed to making their stay here so fun and rewarding. A second group from Western Michigan University also spent a week of vacation here. The Eastern Fat Tire Association race winds through the beautiful and diverse terrain of Greenfield, Francestown, Hancock, Bennington, Deering and Weare, traversing a portion of Crotched Mountain near the end of the course. “We are thrilled to welcome the Eastern Fat Tire Association back to Crotched Mountain for their second annual race,” said vice president of development Shari Landry. “Proceeds of this fundraising race will support rehabilitation services for the children and adults with major disabilities, brain injuries and related conditions we treat here at Crotched Mountain.” Novice, Sport, Expert and Pro/Elite classes for men and women will be represented in the race that begins and ends at Oak Park in Greenfield. Campsites for the participants will be available. To register online, contact www.active.com. A LONG, LONG WINTER We know. The winter was rough on everyone. But consider the maintenance crew at Crotched Mountain who, with an average of one snow storm a week, maintained roads and walkways campus-wide—after every storm clearing approximately six miles of road, even more miles of sidewalks, ten parking lots and 300 doorways. They accomplished it working 14-hour shifts with four plows and one backhoe.
